That procedure somehow worked for me - I have one of those "capture anything you hear" programs on my computer, captured to MP3, then imported into iTunes, converted to AAC, renamed to M4R - that is where it got dicey. I was really used to the folder view on the left of the old iTunes, but I was able to get the M4R into the ringtones section of the iTunes library, then it was a matter of syncíng the phone. BE CAREFUL! Make sure syncíng is set to manual or it will do the whole thing - which is a pain in the rear for me.
