You might try restore to factory settings, and if you continue to have issues, make a genius appointment and tell them your problem. You may need a new phone. On my 4, after I had it for awhile, I noticed it ran very hot. They gave me a new one under AppleCare.
I have had a lot of issues with my 5 and iTunes and at one point the Apple telephone techs wanted me to go back to factory settings. They said there could have been a problem in the original sync. In the end, though, all I had to do was re-download 4 tunes. So far.
