How are you getting a dBm reading? Since most of us just quote number of bars? I'm serious with the questions as I often work literally at "wide spots in the road" or "some farmer's field".
Call ATT at 611 and ask them to review the set up on your account, specifically which phone is showing on the account and the related settings.
I moved to a iPhone 4S in early July and when I called in with a similar issue in mid-October, they still showed me as using a 3GS. They reset the settings and over the air updates some settings on my iPhone and I have not had a re-occurance.
Funny, I was out of state for the first 90 days, and this did not raise its head once.
