I think this is an AT&T capacity issue. When their towers have more traffic than they can handle, some of them are not dispatched but sent to voicemail. This happens occasionally due to tower congestion. I have it sometimes with my 4S and my wife gets it with her 3GS. The only remedy I know is to switch to Verizon.
By the way, are you in a huricane Sandy affected area? This would likely exacerbate the problem.
