I suspect it is the external batteries that are the problem. If Chas has not already, and there is a nearby Apple Store, I would take it in and have them check it.
The ChargeIt! brand seems a little flaky, since I can not find their site, and none of them give you any specs on the mAh. That and the price being that low, I can not imagine it has that many mAh.
If you look around the web, you will find that if you leave a external battery like a Mophie connected all the time, that you get less charge out of it, since it is the last bit of the charging of the iPhone that wastes the most of the mAh. And this is what it sounds like he is seeing. That sudden drop after removing the external battery, sounds like the software battery charge gauge correcting itself. They have never been known to be accurate.
Of course the other side of the coin is to use up the iPhone charge (and eventually the iPhone battery) first, then use the external battery. To my way of thinking, I would rather use up the finite number of recharges on the external battery, before the iPhone battery's. But then again, I buy the AppleCare, when I buy a new iPhone.
If it seems too good (cheap) to be true, it probably is. My suggestion is for him, if the duration of his iPhone charge is that important to him, is to go out and spend the $90 plus and get a Mophie or another reputable brand. And yes, I am a satisfied, repeat Mophie customer, but nothing more.

But judging by your story, I think you need to try a whole cycle charge (drain-full charge-drain) first and see if it's still the same.
