Yes it is possible. I saw someone did that. You have to open the back of your iPhone. Most replacement batteries you can buy from eBay or Amazon come with a compatible screw driver.
  
  But judging by your story, I think you need to try a whole cycle charge (drain-full charge-drain) first and see if it's still the same.
