I had a similar problem a few days ago after a visit to the Apple store to see about a phone replacement because people were complaining they couldn't hear me. The Genius bar tech gave me a new sim and reset my settings. I could no longer send/receive from any device other than my iPhone. I went into Settings -> iMessage -> Send & Receive and made sure that my phone # and my AppleID were both listed. Once I added my AppleID I then got a popup message on my iPad and my iMac that phone # was added to iMessage and FaceTime. Everything was then working as it was supposed to.

One thing to keep in mind. If you have 2-factor authentication setup on your AppleID you might have to generate a new app specific password for iCloud/iMessages/Facetime on your iPad & Mac before all works seamlessly again. I was forced to do this. I was able to use the same password for iCloud/iMessage/Facetime on each device.

I was on the phone for over 2 hours today with Verizon and then with an Apple Tech who passed me off to a supervisor who promised to call me back but did not. The situation is not resolved.

I am sure there must be a setting that I have missed butI can't seem to find it and neither could either of the tech I talked with today.

Does anyone out there have any ideas about this situation?

My MacBook Pro is a 13" with Yosemite V10.10.3
IPad is iPad 3 - ios8.3
iPhone is iPhone 6 with iOS 8.3
