To find the app on the iPhone, click near the top of the screen and pull down. A search window should open. Type in the name of the app, and you if it is on the iPhone, you should see the app and the folder that it is in to the right. Then follow Otto's instructions to delete it.
If the App is still in the setup for that iPhone, in iTunes on your computer, it might reinstall it, so the next time you sync your iPhone, make sure to remove the app there also.
