The exchange, once authorized, happens automatically.
For most apps, there is a preference in the health app that needs to be set and one in Apple's Health app as well.
In Pedometer, for example, there is a preference to Merge Apple Watch Data that needs to be set to merge data with the iPhone (and watch if you have one). In the Apple's Health app, you select Data Sources and then turn on Pedometer (or MyFitnessPal or Cardiogram, etc).
So a connection from both Health and the other app.
