Just a couple of days ago I made a new ringtone on my Mac, followed instruction to install it on my iPhone (latest version of iOS), and it works fine...

I downloaded an appropriate .mp3, used iTunes to convert it to AAC (.m4a), changed the suffix to .m4r, connected the iPhone to to the Mac by cable, located the .m4r file, located the iPhone in the left pane of iTunes, and drag-and-dropped the file onto the iPhone icon.
