Rebooting solves many issues.

I get the folder it is in to wiggle, then tap the circled x and get the Delete or Cancel message. I choose the Delete option and it is still there. I've tried deleting in iTunes but same results. I click on the icon, it gets surrounded by the blue box but no circled x appears to delete it.

Sometimes I sound like I only know one tune, with my advice, but:
I would try re-booting (press and hold both the Home and On/Off buttons until the Apple logo appears, then let the re-boot complete). Then try the delete again.
