I just had an icon on my iPad mini that was not associated with any app. It had no name on it and was a very basic design with a circle and X across it. I could not delete it on the iPad. I hooked up to iTunes and moved it to a different page and did a sync. After the sync it was gone.

Connect your iPhone to iTunes.
Then use the apps section of iTunes to uninstall the app.
Then click apply to sync your iPhone.
That should remove the icon & the app from your iPhone.
