The "thing going on" could have to do with iCloud synchronization. Like with a poor connection (now or before) resulting in corrupted data.
Again, not /the/ answer, but a reasonable guess.
(Me, I lost draft e-mail messages on one of my iOS devices due to Google's server trouble in California and I may well have lost some One Note notes with Microsoft, even though I'm in Europe.)
