The old ones used to go bad when I squeezed the lightning connector end slightly while removing. I haven't done a post mortem, but I suspect the early ones were "chip on board" construction and squeezing broke the delicate bond wires.
Given the cost of the things, I treat the one remaining cable that works like an eggshell. Instead of squeezing on the wide part of the lightning connector, I press gently from the sides. So far so good. I also coil it with the same diameter as the original packaging to avoid stress to it.
