Are you sure "Do Not Disturb" has not been activated?  If it is, you will see a "crescent moon" icon in the top of your screen.

I have the same issue   -   when my phone is locked   black screen  -  I do not get text notifications at all.

Sorry but I need to ask the obvious: have you (unknowingly) flipped the mute button on the side? I find this tends to happen when removing and replacing the case. (This doesn't explain the lack of vibrate, though..)
