When we were on AT&T, we had a similar problem, though we don't live in a rural area. It was just a weak spot in their service. Like Pat, we ended up with a microcell, which solved the problem completely. There was a purchase price (which the rep I spoke with managed to make almost "free" by lowering our monthly bill for several months), but there was no monthly charge, at least at that time.

I see from the AT&T site that the microcell presupposes a separate broadband connection, by fiber or by coax, to which to connect your calls. It probably would not give satisfactory results with a slow DSL connection over copper wire.
