Which app did you add it to?
I assume that I can simply dragged the file into iiTunes or used File > Add to Library and let iTunes do the rest. I suspect it didn't actually get synced to the iPhone because the format wasn't quite right.
Ian, did you convert the file again in Handbrake, using the iPhone preset (click Toggle Presets in the toolbar if presets aren't visible)?
