My experience with my iPhone and my car (Infiniti) is like yours. I get in the car and they simply connect up (with BT on and pre-linked for both the car and the phone). This includes hand-free telephone, streaming internet radio or music from the iPhone itself.

I didn't have to do anything special with the iPhone. Bluetooth in ON in both the car and the iPhone, and it just connects as soon as the car is turned on.
I suspect you may be suffering either from a bad implementation of BT in your car, or an out-of-date version.
You might try a web-search for "bluetooth in <my car>" and see what turns up.
