I am not totally up to date on cellular things, but... with a quick web search for RUIM, I found the following:
"Removable User Identity Module (R-UIM) is a card developed for cdmaOne/CDMA2000 ("CDMA") handsets that extends the GSM SIM card to CDMA phones and networks. To work in CDMA networks, the R-UIM contains an early version of the CSIM application. The card also contains SIM (GSM) application, so it can work on both networks. It is physically compatible with GSM SIMs and can fit into existing GSM phones as it is an extension of the GSM 11.11 standard.[1]" from:
http://en.wikipedia.org/wiki/Removable_User_Identity_Module
I was surprised to find from reviewing the previously suggested Wiki page, that there are 5 cellular providers in the US that provide both GSM and CDMS. But the page does say that the information might be out of date.
Thank you for informing me on the existence of RUIM cards.
Have you tried contacting Sprint or Verizon for your answer?

Traditionally cdma does not have sim cards. But now they've started giving
RUIM cards which are similar to gsm sim cards. For iphone cdma part is
programmed in the phone. US cellular in USA has started giving RUIM card
based phones along with cards.
