In my Settings/Sounds is a slider for ringer volume, and a toggle to determine whether or not you can change the ringer volume with the volume buttons..
What are your settings here?
As verified by the AppleCare Tech both were in the correct position. But the tech failed to have me turn the iPhone OFF then back ON. That cured the problem.
