What I do is create folders on my computer labeled with subjects that make sense to me. I then use photosync to transfer new photos I have taken on my iPhone or iPad wirelessly to the computer and put them in their appropriate folders - or I can use the sync cable to access the camera roll on the phone and manually transfer the photos. Once I have the pictures in their proper folders organized the way I want, I connect my phone to the computer and through iTunes I check off the folders that I want synced to the phone or iPad. Each device can have different folders synced or you can put the same photos on both devices.

When you take a photo on the phone or ipad you can then use the above method to transfer that photo to the computer in its appropriate folder and it will sync back to your organized albums on your phone or iPad.
