I also keep my iTunes library on an external drive, Drobo (RAID) to be exact, and point iTunes at it. No, I do not back it up.

Early on, when iTunes was new, I vaguely remember loosing something. I think it was a movie, so it might have been compatibility. I honestly don't remember. I do remember having some difficulties with my audiobooks back then. But again, that was close to a dozen years ago.

One issue you need to deal with if you keep your iTunes library on other than your boot drive is that you must make sure that it is available and awake before you access iTunes in any way. The Drobo like to go to sleep. If not iTunes.app will create a new blank library on the boot drive. If you are not careful, any new additions will be stored on the boot drive library and not your chosen library. The solution for this easy enough, you copy or drag and drop the contents from your boot-drive library to your chosen library, and change the path in the preferences.

The above might explain some of the missing iTunes content.

I also keep my audiobooks and e-books in folders at the same level as iTunes (folder), below Music (folder). I keep my movies at the same level as the Music (folder).
The structure of my Music and iTunes library folders without my changes is a mix of the prior iTunes library structure and the current one. At least until OS X 10.7.5.

So other than a few minor losses, in the early days of iTunes, I have had not loses, and I am happy with iTunes. I am not a big music listener, but my iTunes folder is about 340 GB. In the past I have worked with an Apple engineer to try to fix the wandering pointer, but Apple chose to go with streaming from their servers instead. I like my stuff on my hardware.
