I just went through this. I am with AT&T. I had to go the the local AT&T store, they replaced my sim and activated the iPhone right there. You have a new phone so all you need is the IEMI & sim numbers and you can call AT&T via land line and they can do it over the phone.
I'm not sure about other carriers, perhaps a CDMA iPhone can be activated by call the carrier from a land line.
