The remainder of the items are not filled in, but I've never had to do that before so I don't want to make things worse by guessing what to enter. I turned off my phone (all the way) for over a half hour and it's still the same.
One possibility to try is:
iPhone - Settings - General - Reset - "Reset Network Settings"
