Settings - messages - send as SMS - off

Will not allow sending as SMS at all.

Even with this on I get failures all the time of receiving party not
being able to receive iMessage and asking if I want to send as SMS /
mms. And I have unlimited messaging on AT&T. Unless feature acts
differently on Verizon / sprint.
