sounds like you have Assistive touch turned on. go to settings/accessibility/Assistive touch. if it's on, turn it off..

I must have clicked on something accidentally and now I have a square black symbol with a white circle in the center that shows up on every screen on my iPhone 6S+. If I click on it there are a bunch of different functions I can select. It always seems to be in the way and I find myself clicking on it accidentally. I just want it gone. Any ideas what it is and how to get rid of it?
