Not sure how it got turned on. Try Settings, General, Accessibility, and turn off Assistive touch.

I must have clicked on something accidentally and now I have a square black symbol with a white circle in the center that shows up on every screen on my iPhone 6S+. If I click on it there are a bunch of different functions I can select. It always seems to be in the way and I find myself clicking on it accidentally. I just want it gone. Any ideas what it is and how to get rid of it?
