I had the iCloud address on both the iPhone and the iPad and iPad messages still didn't get sent to the iPhone. Then I switched the "send" address on both machines from my regular email address to the iCloud address. Now all messages appear on both devices.
A friend on another list uses his mac.com address. Looks like iMessage prefers the "send" address to be Apple generated. Odd it's not mentioned anywhere I've seen.
