I started using photo stream recently. And I also get duplicates in iTunes and on my desktop, although that seems random to me. I also use hi def setting on my iPhone settings and that automatically gives you a duplicate one in regular mode and one in High def mode. I just found that out.
iTunes too is enigmatic at times. Recently and I am not sure what happened all my pictures that I had organized suddenly were unorganized and put into one big file. For weeks I have slowly been reorganizing them into folders and I'm so afraid the same thing will happen in the future. I have a lot of duplicates on my computer and one of these days I'll pull them off and put them in another hard drive. problem with that is that I rarely go back and look at my extra hard drives, but at least they are there as long as the hard drives don't fail. I'm starting to think that unlike "real" photos nothing is forever. At least with real photos I can put them in an album and there they are if I want to look at them. But again, it is starting to become a think of the past.
Other than that I like iTunes, but it isn't perfect or near perfect and I hear or read from others that there are other programs they like better. I don't remember what they were.
I think you can choose which pictures you want on you iPhone or iPad. I don't know if I went about this the most efficient way, but I put a new folder in iTunes that says iPhone pictures and one that says iPad pictures. When I sync the iPhone for example you can go into the menu at the top where it says pictures (I think I am not into it right now) and then I check off the folders I want to sync with that device. I think you can also set up an iPad folder and then when your iPad is syncing in your computer you can choose that folder and/or other folders you may want to sync. I have a folder on my iPhone that says iPhone pictures, but what I found is that no matter which folder I choose there is still a folder that says library and it has almost the same pictures I put into my iPhone folder.. I haven't quite figured that one out yet. but I think once you have specific folders you can put in the pictures you want and then delete the picture in the camera folder.
