It has nothing to do with your phone or your service. 

I would be a little concerned that the spoof caller had chosen to call your doctor, or others on your contact list. That is one heck of a coincidence. I would double check that it was not pocket dialing.

In addition, I've also had a problem with random calls from people saying that I called them.  My phone as well as AT&T's records show no calls going out to these numbers so no pocket dialing.  AT&T told me that someone was using my number with a "caller ID spoof" app which allows someone calling and having my number coming up in the caller ID.  

Is this a problem with my phone or my AT&T service? Anyone else have odd problems with their phones? BTW, I have an iPhone 4.
