There is no such thing as a CDMA SIM card.
For the iPhone, I understand, Verizon provides a SIM card for their iPhone so that it can use GSM internationally, but the phone is CDMA for US service. I believe the Verizon iPhone is the only phone that can do both. The AT&T iPhone can only do GSM voice.

Just remember GSM, SIM card. CDMA, no SIM card.
