The default Camera app doesnt appear to have any settings that can be set that will cause fuzzyness, such as altering the shutter speed or F stop so as to cause a low ISO. But could she be using a different camera app where a manual setting has been set? I guess not, but worth asking

Did you get Applecare+ when you bought it? It should be no more than 2 years old, and still covered if you did.

Get a flashlight and look at the camera lens (the fuzzy one) with it. You may be able so see some cloudiness or droplets if they are present.

If you have an Apple Store in reasonable distance, it would be worth asking there. Figuring it out would probably be free, and who knows, maybe fixing would be free or reasonable..
