I also failed to mention that I have one of 6 iPhones here, my entire family
has them. Three 4Ss and three 5's. I am the only one with this problem.
Many times when I can't get mine to load anything, my husband, who is with
me, can.
I always show 3G, no matter where I am. Most of the time I have full bars
too.
My Cellular Data switch under Settings is ON.
Verizon and Apple have walked me through about every setting change that can
be made, as well as two restores.
I think it's time to ask for a replacement phone.

Janet, if your Cellular Data setting is on, then I think the problem may be
your location. You say your are rural. Where are you, when you are not
receiving a data signal? At that time and place, how many bars of signal do
you have?
From your ISP, you appear to be between Eau Claire & St Paul. Looking at
satellite map from google, there are lots of hills and forest south of
Durand. I've been in the Ellsworth area for work and I remember the cell
signal was spotty in the area. Remember that cellular signal is somewhat
line of sight, meaning it will not go thru hills or drop into the next
valley.
The spinning at the top of your screen is telling you that it is searching
for a signal. When you are away from your home WiFi, do you ever see one of
the following icons: 4G, 3G, E or º (like a degree symbol)? When you are in
a spot with a good cellular signal, like were you get full bars, turn off
the WiFi in settings. Check and see if you have any of these icons. Then
check to see if you can access any of the internet apps.
WiFi has nothing to do with cellular signals. Verizon telling you to turn
off the WiFi will do nothing for your cellular signal, but since you are
rural and there is no WiFi signal in the area, it will help to save your
battery charge. When a cell phone searches for WiFi and/or cell signal, it
uses a lot of power.
At this point, my guesses are either you are not receiving a cellular
signal, because of your location, or your phone is defective. I don't know,
but I think if you are getting cellular voice, the cellular data part is
working, too.

1. If you have no LTE in your area, then there is o problem with leaving
Enable LTE turned on.
2. Is there a "Cellular Data" switch under Settings -- General ?
