Sam, I've had an Iphone since ios4 but I've never seen a badge on the
settings icon, and I wouldn't have said no if I'd ever been asked if I
wanted a badge.

If you have a badge on the Settings icon, why do you need another icon and
badge? Just check out why something is trying to get your attention in
Settings. Follow the badge down, until you find out what it is trying to
tell you.

This feature, OTA, came out with iOS 5.0 in October 2011. It has been used 4
or 5 times since it came out, or on average every 3 months. Although, the
last one was May 2012, but you seemed to have figured it out.
