The great majority of iPhones sold in the USA are sold locked to a single carrier. That is their normal state. (And that is the normal meaning of the term when used with iPhones.) They work fine; they do not need to be unlocked, in normal usage of that term, to be used.
Current news is that ATT will unlock or allow unlocking for ATT iPhones that are now out of contract, so their owners can obtain a different SIM card and use them on a different cellular network.
If this is NOT what you mean, are you inquiring about an iPhone that has a PIN code locking it? And someone has lost their PIN and can't unlock? If so, that is a very different problem.
And if this is the problem, they had better have a recent backup, because it is unlikely they can fix this without wiping the iPhone clean.
