Speaking for myself:

An international plan is not *required* for using any 3G service, but
it is likely to be cheaper.

I live in the Netherlands, and I happen to have all my traffic with
T-Mobile NL, a subsidiary of T-Mobile in Germany.
I have a smartphone (not an iPhone) and an iPad.

I could add an international plan to either subscription.

I could also buy a short-term international ticket.
First, I'd have to activate this option on my profile page.
Then, upon crossing the Dutch border, I'd receive a text message
inviting me to buy such ticket by texting back or by visiting a
website.
Pricing depends on duration and zone (distance). It seems to cover the
cost of roaming.

Note:
There is the gamble of how much one actually uses (minutes, messages,
MB), so what would prove the best approach with hindsight.
