When it happened to my sister with the iPhone 4 I gave her when I upgraded I searched the problem & ended up on a Apple Forum it listed things like iOS upgrades & visits to genius bars & outright replacement as a result (lucky they were under warranty or Applecare) but the posting that helped the most was right near the end & explained the contacts between the speaker & the motherboard get a bit corroded or something and to place the phone on a firm surface and firmly press around the speaker with your thumbs yes being careful not to press so hard that you break the glass but you can press very firmly the "glass" appeares to flex a little & I assume the speaker below moves as well, subsequently rubbing the contacts back & forth on the board below breaking thru the light layer of whatever it is & reconnecting, for me I fixed the iPhone on about 3rd of Nov & its still working a month later.
Same thing with the phone it was well cared for always kept in a pouch on a belt not in a pocket used sparingly against the ear (bluetooth connected) etc.
